Direct Deposit Request Form

Excel Partners realizes the importance of receiving your pay as quickly and conveniently as possible. In order to achieve this, we accept two forms of electronic payment which are described below. Please choose which method you prefer and return this form to us prior to starting your assignment. You may change your selection at any time.

Federal W4 with Instructions

Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.

CT W4 with instructions

Employee’s Withholding Certificate, provides your employer with the necessary information to withhold the correct amount of Connecticut income tax from your wages to ensure that you will not be under-withheld or over-withheld.

NY W4 with instructions (NY IT2104)

Employee’s Withholding Certificate, provides your employer with the necessary information to withhold the correct amount of New York income tax from your wages to ensure that you will not be under-withheld or over-withheld.

The Immigration Reform and Control Act of 1986 (IRCA) required employers to verify that all newly hired employees present "facially valid" documentation verifying the employee's identity and legal authorization to accept employment in the United States. The I-9 form or more properly the Employment Eligibility Verification Form is provided by the federal government for that purpose.
